EDUCATIONAL GAP AND NEEDS ANALYSIS STATEMENT: Gynecologic disorders are commonly diagnosed and treated by Primary Care Providers. Primary Care Providers must maintain adequate knowledge to diagnose and effectively treat commonly seen Women’s Health and Gynecologic conditions, many with considerable morbidity and even mortality if not properly diagnosed and treated. Needs analysis specifically identifies the ability to diagnose Menopause, AUB, Osteoporosis, Hirsutism, and Thyroid Tests as vital skills for the Primary Care Provider. Educational gap analysis has identified two areas that must be addressed: increased knowledge and increased confidence on the part of Primary Care Physicians in the specialty of Gynecology. To address these needs and close these gaps, an annual update, presenting current thinking and new information, will help Primary Care physicians obtain adequate knowledge and provide them with the confidence to diagnose and treat their patients’ Gynecologic and Women’s Health conditions.

Day 1 Amenorrhea. Hormonal Contraceptives: Good, Bad and Controversial. Abnormal Uterine Bleeding. Anemia – A Finding, Not Yet a Diagnosis.
Day 2 Chronic Anovulation. Osteoporosis: Treatment and Prevention. Menopause: Myths and Realities. Migraine Headaches – Update on Diagnosis and Treatment.
Day 3 Hirsutism. Premenstrual Syndrome. Evaluation and Initial Management: Benign Breast Disease. Gynecologic Complimentary Therapy.
Day 4 Treatment of Sexually Transmitted Infections. Ectopic Pregnancy. Venous Thromboembolism (VTE): Diagnosis and Treatment. Pelvic Pain.
Day 5 Health Screening and Immunizations – EBM. Gynecologic Emergencies. Use and Interpretation of Thyroid Tests. Endometriosis.

Primary Care and Women’s Health: An Update and Review

OBJECTIVES

OVERALL OBJECTIVES: The overall objective is to provide the participant with practical and clinically relevant information. Upon completion of this CME activity, the physician or healthcare provider should be able to describe the current approach to formulating differential diagnoses, diagnostic, therapeutic, and preventive management of the various disciplines presented.

SPECIFIC OBJECTIVES
Day 1Amenorrhea. - Upon completion of this session, the participant should be able to:
1.Assess the various causes of primary and secondary amenorrhea and their associated health risks, as well as apply the appropriate evaluation for both.
2.Provide therapy to the patient with regard to hormone replacement therapy or infertility.
Hormonal Contraceptives: Good, Bad and Controversial. - Upon completion of this session, the participant should be able to:
1.Describe clinical differences in oral contraceptives.
2.Decide the appropriate agent for specific patients.
3.Give attention to relative and absolute contraindications to oral contraceptives.
Abnormal Uterine Bleeding. - Upon completion of this session, the participant should be able to:
1.Define abnormal uterine bleeding.
2.Differentiate between functional and organic causes.
3.Manage most non-surgical etiologies of this problem.
Anemia – A Finding, Not Yet a Diagnosis. - Upon completion of this session, the participant should be able to:
1.List most common anemias in women and their usual etiology.
2.Compare the diagnostic evaluation needed to determine the etiology of the anemia.
3.Identify the therapeutic options for each of the identified anemias.
4.Describe the follow-up plans for assessing the efficacy of treatment for each anemia.

Day 2Chronic Anovulation. - Upon completion of this session, the participant should be able to:
1.Discriminate between the hypothalamic, pituitary and ovarian causes of anovulation.
2.Investigate associated health risks.
3.Formulate an appropriate treatment regimen.
Osteoporosis: Treatment and Prevention. - Upon completion of this session, the participant should be able to:
1.List the risk factors associated with osteoporosis.
2.Discuss the current methods for evaluating patients at risk for osteoporosis.
3.Describe the various therapeutic classes of medications and their appropriate use.
Menopause: Myths and Realities. - Upon completion of this session, the participant should be able to:
1.Classify the various issues that face the menopausal woman.
2.Differentiate between true concerns and current anecdotal myths.
3.Discuss topics such as: osteoporosis, cardiovascular disease, breast cancer and endometrial cancer as well as their current therapies.
Migraine Headaches – Update on Diagnosis and Treatment. - Upon completion of this session, the participant should be able to:
1.Discuss the process for evaluating a patient with possible migraine headaches.
2.Discuss the advantages of supportive therapy and lifestyle changes to treat migraine headaches.
3.Compare and contrast the pharmacologic options for treating an acute migraine headache.
4.Identify when prophylactic therapy is appropriate for patients with recurrent migraine headaches.

Day 3Hirsutism. - Upon completion of this session, the participant should be able to:
1.Understand the pathophysiology of abnormal body hair growth in women.
2.Differentiate the source of androgen excess.
3.Review current treatment options.
Premenstrual Syndrome. - Upon completion of this session, the participant should be able to:
1.Identify patients with this disorder.
2.Provide education for the patient as to the myths and realities of the premenstrual syndrome.
3.Review current effective therapies.
Evaluation and Initial Management: Benign Breast Disease. - Upon completion of this session, the participant should be able to:
1.Describe benign breast disease.
2.Evaluate for various conditions such as breast pain, breast lumps, and nipple discharge.
Gynecologic Complimentary Therapy. - Upon completion of this session, the participant should be able to:
1.Define various forms of complimentary medicine such as acupressure, acupuncture, herbal therapy, massage therapy, etc.
2.Answer general questions from patients regarding various complimentary therapies.
3.Understand the basic mechanism of action with various complimentary therapies.
4.Apply complimentary therapies to various gynecologic problems such as PMS, infertility and ovulation disorders.

Day 4Treatment of Sexually Transmitted Infections. - Upon completion of this session, the participant should be able to:
1.List the treatable forms of STDs.
2.Compare and contrast the diagnosis of STDs.
3.Identify the treatment of options for treatable STDs.
Ectopic Pregnancy. - Upon completion of this session, the participant should be able to:
1.Recognize an early extrauterine gestation and intervene or facilitate intervention before an emergent situation exists.
2.Discuss non-surgical management of Ectopic pregnancy.
Venous Thromboembolism (VTE): Diagnosis and Treatment. - Upon completion of this session, the participant should be able to:
1.Describe the sensitivity and specificity of the diagnostic tests available for the identification of DVT.
2.Explain the algorithm for diagnosing a PE in various clinical settings.
3.Compare the role of inpatient and outpatient treatment of DVT.
4.Describe the proper monitoring and adjustment of warfarin therapy for DVT/PE.
5.List the indications for DVT prophylaxis.
Pelvic Pain. - Upon completion of this session, the participant should be able to:
1.Assess the patient with acute or chronic pain.
2.Identify the differential diagnosis and choose appropriate management options for the patient.
3.Identify true surgical emergencies.

Day 5Health Screening and Immunizations – EBM. - Upon completion of this session, the participant should be able to:
1.Describe the evidence based recommendations for preventive screening tests for adult women.
2.List the routine immunizations recommended for adult women.
3.Describe the common side effects of adult immunizations.
4.List the contraindications for each adult immunization.
5.Discuss why and when screening tests can be discontinued in adult women.
Gynecologic Emergencies. - Upon completion of this session, the participant should be able to:
1.Recognize, provide initial management, and identify when to refer emergent scenarios in women’s health care including:
a.Abnormal pregnancies (ectopic vs. miscarriage);
b.Menorrhagia;
c.Thyroid storm;
d.Abdominal trauma in pregnancy.
Use and Interpretation of Thyroid Tests. - Upon completion of this session, the participant should be able to:
1.Compare the utility of the various serum tests for thyroid function.
2.Distinguish the combination of results that occur in clinical settings.
3.Describe the appropriate follow-up of abnormal thyroid tests.
Endometriosis. - Upon completion of this session, the participant should be able to:
1.Have a high degree of accuracy in the suspicion of endometriosis based on symptoms and physical examination.
2.Review current management options with particular attention toward agents that can be effectively used by the primary care provider.
