OVERALL OBJECTIVES: The overall objective is to provide the participant with practical and clinically relevant information. Upon completion of this CME activity, the physician or healthcare provider should be able to describe the current approach to formulating differential diagnoses, diagnostic, therapeutic, and preventive management of the various disciplines presented.

NEEDS STATEMENT: Controlling costs while promoting preventive care and a healthy population is an immediate challenge for all Primary Care Practitioners. There have been recent changes in Standards of Care for Diabetes and DVT as well as New ATS and IDSA Guidelines published for the treatment of CAP. This course will provide this needed review and update. The increasing incidence of Asthma with corresponding rise in mortality as well as improved approaches to COPD therapy indicate that annual updates are needed. Additionally, the increasing use of Bariatric Surgery and the resulting Nutritional issues arising from these surgeries require an update in this area as well. This timely CME information will help to improve patient outcomes and the overall health of patient populations.

SPECIFIC OBJECTIVES
TOPIC 1Asthma - Upon completion of this session, the participant should be able to:
1.Describe the epidemiology of asthma.
2.List the various modalities to diagnose asthma.
3.List the different treatments for asthma, both pharmacological and environmental.

TOPIC 2An Approach to the Dizzy Patient - Upon completion of this session, the participant should be able to:
1.Review the common presentations of dizziness.
2.Describe the pathophysiology of vertigo and nystagmus.
3.Apply bedside techniques to distinguish dizziness subtypes.
4.Identify and apply potentially curative treatment for benign positional vertigo.

TOPIC 3Type 2 Diabetes - Upon completion of this session, the participant should be able to:
1.Describe the new American Diabetes Association recommendations for screening for diabetes.
2.Describe the current approach to drug therapy for patients with Type 2 Diabetes.
3.Define the target levels for blood glucose control and hemoglobin A1C.
4.Describe the preventive health measures particularly for patients with Type 2 Diabetes, including screening for microalbinuria, diabetic foot exam, and ophthalmologic exams.

TOPIC 4Community Acquired Pneumonia - We will review the diagnosis and treatment of this common disorder with emphasis on the published guidelines. Upon completion of this session, the participant should be able to:
1.Categorize patients with respect to severity of illness.
2.Develop a strategy to triage patients - outpatient therapy, hospitalize or ICU.
3.Characterize the different pathogens that commonly cause community acquired pneumonia.
4.Identify the current status of diagnostic work-up including the use of cultures, gram stains, serology and bronchoscopy. We will emphasize a cost effective approach.
5.Demonstrate how to properly use antibiotics for community acquired pneumonia. We will discuss both old and new agents.
Numerous cases will be presented with chest x-rays and gram stains to illustrate important teaching points.

TOPIC 5COPD: Overview of Patient Management - Upon completion of this session, the participant should be able to:
1.Derive an understanding of the definition and pathophysiology of COPD.
2.Characterize the natural history and clinical features of COPD.
3.Outline the adverse health effects of cigarette smoking and present recommendations for helping patients “kick the habit”.
4.Identify the many aspects of outpatient management including long term oxygen therapy and pharmacologic treatment. We will present a treatment algorithm and discuss indications for specialist referral.
5.Review surgical treatment options for emphysema.

TOPIC 6Deep Venous Thrombosis and Pulmonary Embolus - Upon completion of this session, the participant should be able to:
1.Determine when to use V/Q scans, pulmonary angiography, and non-invasive flow testing in the work-up of suspected pulmonary embolus.
2.Interpret the results of these tests according to the pre-test probability of disease.
3.Describe the indications, contraindications, and therapeutic regimens for outpatient management of deep venous thrombosis with low-molecular weight heparin.
4.Describe how long anti-coagulation treatment should be continued after deep venous thrombosis.

TOPIC 7Cobalamin (B12) Deficiency and other Neurologic Disorders of Gastrointestinal Dysfunction - Upon completion of this session, the participant should be able to:
1.Explore recent advances in recognition of B-12 deficiency and Post-Gastroplasty Neuropathy.
2.Examine the changing clinical presentation of this illness.
3.Apply newer laboratory techniques to the diagnosis.
4.Use an algorithm for the diagnosis and treatment.
